Child and Adolescent Intake
Dr. Stout will meet with the patient and caregivers, obtain information from school, therapists, and gather data to make a clear diagnosis. We will work together to determine a treatment plan and if medications are necessary.

Adult Intake
Dr. Stout will meet with you and any loved ones you choose to include to discuss your strengths, struggles, concerns and barriers to living your best and most fulfilled life. We will discuss potential diagnoses and treatment options to make a plan together on a treatment plan
60 mins | $400
Follow up
Evaluating the treatment plan and medications to ensure everything is working well for you. Follow ups can occur once every 1 to 6 months depending on your overall progress
